This five-week "Hemodialysis Training Course" is designed to equip participants with the knowledge and skills necessary to provide hemodialysis treatment and efficient care to renal patients.
At the end of the course, participants should be able to:
@Apply the principles of hemodialysis in a dialysis facility;
@Operate hemodialysis devices, water systems and reprocessing machines;
@Practice infection control;
@Provide patients with safe and efficient care;
@Safeguard patients' vascular accesses;
@Coherently discuss hemodialysis with patients.

CEU/ CONTACT HOURS/ CREDITS - USA

FOR NURSING LICENSE RENEWAL IN THE UNITED STATES


A "contact hour" credit refers to 1 hour (50-minute hour) of instruction or 1 hour of self-study (home study). 30 contact hours equals 30 hours of instruction or 30 hours of home study.


A "CEU" refers to "Continuing Education Unit." 1.0 CEU = 10 contact hours, (hours of instruction) or 10 contact hours of home study (self-study).
In California nurses need 30 contact hours, OR 3.0 CEUs every two years. Many people use the terms "CEUs" and "contact hours" interchangeably. However, 30 contact hours is the requirement in California and other states. (30 contact hours = 3.0 CEUs)


Alabama State Board of Nursing requires 24 contact hours, 24 allowed by home study; limit of 12 contact hours by independent study. "All other CE, including videos, Internet, etc., are considered attended." (Online renewal--The nurse must first go to their "Individual Record" on the BON web site; then add the CE course. Then the nurse will be able to renew their license online using any one of our CEU provider numbers listed on our certificate of completion.
Alaska State Board of Nursing requires 30 contact hours, all allowed by home study.
Arkansas Board of Nursing requires 15 contact hours, all allowed by home study.
California Board of Registered Nursing requires 30 hours every 2 years (all 30 may be by home study)California Board of Vocational Nursing & Psychiatric Technicians (same as RN)
Colorado State Board of Nursing no mandatory CEU requirements at this time.
Connecticut Board of Examiners for Nursing no requirements at this time.
Delaware RN Board requires 30 hours, all allowed by homestudy

Florida Board of Nursing requires 25 contact hours every 2 years, all of these may be taken by our homestudy classes. [FOBN also requires classes on HIV/AIDS, Domestic Violence, Prevention of medical errors, and/or End of life topics; must be Florida approved provider only for these topics.]

TRAINING TIPS:

On the first day, they will provide you with a checklist booklet (Nursing Standards on Intravenous Practice) containing different procedures and the steps needed to implement them.

MEMORIZE all these procedures by heart because on the third day or last day of your training you will have a return demonstration of all these procedures.

Be familiar with the materials that are needed in theses procedures because during the return demonstration, you are going to explain the procedure while you are practically doing every step that is written in the checklist. Example - How can you perform an IV push using the 3 way stop cock with luer-lock if you don't know the function and how that device looks?

On the day of the return demonstration, there are preceptors assigned in every procedure. Each procedure has its own demo area, so you will have the freedom to start your demo at your own choice of procedure and at your own pace.

Remember - don't be nervous. Start with an easy procedure, after finishing a procedure you will gain confidence and you can now move on to the other.

After the training, you still have to complete a number of case in order to have your license. Ask your proctors about the procurement of your license.

THE RED RIBBON - CONTINUATION

MORE INFORMATIVE NOTES:

1. Go to PRC and bring also the VERIFICATION REQUEST (FORM 5) Be sure to fill up Part A. Ask PRC to complete the form and pay some fees so that they could send it directly to you.

2. Since you are already at PRC go to this step:Request for a Certified True Copy of your PRC Board Certificate . Pay the required fees and then PRC will FORWARD it to DFA (Roxas Blvd) for authentication. You will be given a claim stub. Then you can claim it at the DFA after 2 weeks.

3. Request for a copy of your Birth / Marriage Certificate at the NSO.
Then bring the documents to the DFA for authentication. Pay the required fees (Php100 per doc) and claim it after 2 weeks.

4. Photocopy the first page of your passport (the one with your passport details) . Go to a legitimate Notary Public and have it Notarized as a Certified True Copy. No need to have it authenticated at the DFA and the Embassy.

NURSING AUTHENTICATION - THE RED RIBBON

Authentication of Nursing Documents is intended for individuals applying to work abroad... The foreign government you want to work under wants to make sure that your documents are authentic, so it wants the DFA to certify it.

Since a lot of those documents are not the responsibility of the DFA, the DFA requires you to have them authenticated by the relevant government department.
WHAT IS THE DIFFERENCE???
CERTIFIED TRUE COPY, NOTARY PUBLIC, DFA AUTHENTICATION

When you say certified true copy it means that you had the original photocopied (e.g. transcript of records, diploma, etc.) and usually there's a dry seal and a signature from an authorized person in an office where you got those documents.

The authenticated documents on the other hand, like the ones coming from DFA for instance they are the ones who make sure that the ceritified true copies of the documents are indeed correct and the authenticating officer will issue you a certification bearing the signature of the officer, a seal and a red ribbon. Also, a documentary stamp is usually visible on the certification.

1) Commision on Higher Education - Located in Ugong, Pasig City, this is your first step. You shall receive official papers from the CHED official that you have graduated in a college or university in the Philippines. Just ask the guards or anyone who works there where the office for TOR and diploma authentication is located. Your transcript of records and diploma shall be photocopied and stamped, and the official papers along with it. This process will take at least a week or two.

2) Department of Foreign Affairs - This will be next in line. You need to submit your CHED-authenticated TOR and diploma.
This is located along Roxas Boulevard, Manila City. Expect a heavy line and a heavy crowd, so you have to be early. Look for its lower ground offices, this is where the releasing of authenticated documents is done. Like the CHED, this usually takes one to two weeks.

3) Red Ribbon in Malacanang - Next is this office. Ask any guard and they shall point to you which office this is located. A red ribbon will be placed around your DFA/CHED- authenticated TOR and Diploma. Goverment clerks will be inspecting your papers and place the ribbon. This usually takes a whole day.

PRC LICENSE CARD/ CERTIFICATION OF RATING/ CERTIFICATION OF PASSING

1) Authenticated by the PRC - All documents pertaining to your Nursing License.

2) Red Ribbon in Malacanang - Next is this office. Ask any guard and they shall point to you which office this is located. A red ribbon will be placed around your DFA/PRC- authenticated Professional Nursing Documents. Goverment clerks will be inspecting your papers and place the ribbon. This usually takes a whole day.

Registration Procedures
You have to register for the test before you can be placed on the list for a particular test date. As these fill up quickly, it is recommended that you register as soon as possible. Be aware that registration for a particular test date is based on payment of the test fee on a first come first served basis. The deadline for submission of application is set two weeks prior to a certain test date, for tests conducted in Manila, and one week prior to test date, for tests conducted outside Manila.
Fill in the Application Form and send it or take it to the test centre with the following

National Council Licensure Examination for Registered / Practical Nurses (NCLEX-RN / PN)

What is the NCLEX Examination?

The NCLEX examinations is designed to test knowledge, skills and abilities essential to the safe and effective practice of nursing at the entry level. NCLEX examination results are an important component used by boards of nursing to make decisions about licensure. Only boards of nursing can release NCLEX examination results to candidates.



The NCLEX examination is administered in the United States, American Samoa, the District of Columbia, Guam, the Northern Mariana Islands, Puerto Rico, and the Virgin Islands. The use of the same examination by all jurisdictions facilitates licensure by endorsement from one board of nursing to another.

Beginning January 2005, the NCLEX examinations will be administered internationally in Hong Kong, Special Administrative Region of the Republic of China, London, England, and Seoul, South Korea. (Taken from

Eligibility Requirements

To take the NCLEX examination, candidates must satisfy these requirements:

? Apply for licensure in the state or territory in which you wish to be licensed.
? Meet all of the board of nursing

Registration Process Overview

1. Submit an application for licensure to the board of nursing in the state or territory where you wish to be licensed.
2. Register for the NCLEX examination with Pearson VUE.
3. Receive Confirmation of Registration from Pearson VUE.
4. The board of nursing makes the candidate eligible to take the NCLEX.
5. Receive Authorization to Test (ATT) from Pearson VUE.

Examination Fee and Registration Information Fee

The fee for taking the NCLEX examination is $200. You may also have to pay other fees for licensure required by the board of nursing in the jurisdiction in which you are applying.

You must check with your board of nursing to find out about these fees.

If you choose to schedule your NCLEX examination at the Hong Kong, London, or Seoul test centers, you will have to pay an international scheduling fee of $150 plus a Value Added Tax (VAT) where applicable. These fees will be charged when you schedule your examination appointment.

Registration Time Limit

All NCLEX examination registrations will be remain open for a 365-day time period during which a board of nursing may determine the candidate?s eligibility. HOW TO OBTAIN THE APPLICATION FORM

First get a copy of the California Board of Registered Nursing Application Form. You will need ADOBE READER to view the file.

APPLICATION FOR LICENSURE BY EXAMINATION

This is one of the first forms that you will need to accomplish basically it includes your personal information. Fill in the necessary information and here are some tips and frequently asked questions I encountered when filling up this form.


ADDRESS:

If you have a relative or family member residing abroad whom you can trust and at the same time ask for little favors then if possible try using a US Address they say that it makes communication faster between you and the Board of Nursing just in case they need something from you. Also it is obvious that mailing time would be faster since letters are only transmitted to the same country as compared to having it mailed in another country. I also say ask for little favors because technically speaking when you mail your forms it would have to come from that US address too so what will happen is that if you are here in the Philippines you will mail the personal forms to your relatives address and they will be the one to mail it to the Board of Registered Nursing… sounds complicated but I hope you get what I mean. If you do not know anyone abroad then just use your address here in the Philippines.


SOCIAL SECURITY NUMBER: Most of us do not have this not unless you can legally work in the US so in most cases you just leave it blank.

Place your 2×2 picture on the second page beside the date on the lower part of the page using tape NOT glue.

Do NOT forget to include your application and fingerprint card processing fee please refer to the application form for the exact amount ($107.00 for application and fingerprint card).

Make sure it is payable to the BOARD OF REGISTERED NURSING.

The rest of the application are pretty much self explanatory so this form has 2 pages.

There is a part about an INTERIM PERMIT which basically says that they give you like a temporary permit to work while while waiting for the exam results which is valid for 6 months or until your results have been released. Personally, I did not avail of it since I won’t be able to use it since I do not have papers to work in the US. So I would rather save the $35.

REQUEST FOR TRANSCRIPT
One of the forms for the California BON is the forms to be given to your school registrar and then they would be the one to submit it through mail to the California BON. As for my tip just try to clarify everything with your school registrar so that less errors would be made in terms of processing.

BREAKDOWN OF EDUCATIONAL PROGRAM FOR INTERNATIONAL NURSING PROGRAMS and NURSING PROGRAM VERIFICATION

Just fill out the parts where it asks you to fill which is basically the personal information and the rest will be filled out by your nursing program. Usually they mail it together with your TOR which was filled by the school registrar.

LETTER OF EXPLANATION or COPY OF LOCAL LICENSE
If you do not have your local license yet just mail them your board certification that you passed which you can request in PRC once you know you passed the exam. And well if you have your license already then just scan it and print it and attach it together with the other documents you will submit.

FINGERPRINT CARD
You would have to request for this separately you can request by going to this link: California Finger Print Card Request

Read thru the instuctions there and wait for it to arrive in your mail. Then you can accomplish the fingerprint in the NBI in Carriedo. I am not sure if it can be done in other NBI branches.

Once accomplished, mail it together with your other forms and DO NOT BEND the fingerprint card so if you can place it in a hard envelope the better. What I did was I improvised an envelope using a folder.

They just need one copy but just in case the print is rejected provide them another copy so you would not have to mail it separately when they ask for another one.

As much as i am giving here tips and guidelines please refer and read the instructions that comes with the forms. There might have been changes in terms of fees and other instructions so it is important to refer to them still.

The processing of my application took about 2 months but then they requested for subject description so it took me another month to accomplish that. But I think the subject description is a case to case basis. In general, waiting period for your application is 3- 6 months depending on the bulk of applicants.

Last tip, if you are to use Fed Ex or Special Couriers in mailing remember that they will not accept PO Box address that comes with the form so you can use this address instead:

DECIDING TO TAKE THE NCLEX HERE IN THE PHILIPPINES???

The NCLEX exam started last August 23, 2007 in the Philippines, this is such a great opportunity that has been given to filipinos. The exam started at 9 o clock in the morning.

To avail the schedule of NCLEX exam in the Philippines you have to call the Asia Pacific Region, in Australia, and their number is 61-2-9478-5400, you tell them you wish to ask for a schedule exam in Makati.

They would be asking for your candidate number and your credit card number and they would be asking you what schedule it is you want to take. Registration is done through the internet while scheduling is done through calling the Pearsonvue contact number.

However, for those second takers who did not pass the exam on first take should have to resubmit an application to the Board of Nursing and pay the fee to Pearson-Vue again to get the ATT. You will be unable to even schedule an exam until you have the ATT in hand.

The NCLEX Examination Process

All exam registration will remain a 365-day time period which a board of nursing may determine the candidate's eligibility.

Candidates who are not made eligible by their board of nursing within the the idol search forfeit their registrations, including the candidate registration fee. Candidates who wish to take an NCLEX examination will need to re-register, including repaying the examination fee, in order to receive an examination.

Students can register for the NCLEX examination and pay the $200 by credit card by calling Pearson VUE at 866.496.2539 or register via the Internet at http://www.pearsonvue.com/nclex.

3. Receive Confirmation of Registration from Pearson VUE.

4. Receive eligibility from the state board of nursing you applied for licensure with.

5. Receive Authorization to Test (ATT) from Pearson VUE.If more than two weeks have passed after you have submitted a registration for the NCLEX examination and received a confirmation from Pearson VUE, and you have not received an ATT, please call Pearson VUE.You must test within the validity dates of your ATT. These validity dates cannot be extended for any reason.

6. Schedule an appointment to test by visiting www.pearsonvue.com/nclex or by calling Pearson VUE.

To change your appointment date:

For exams scheduled on: Tuesday, Wednesday, Thursday, and Friday, call Pearson VUE at least 24 hours in advance of the day and time of your appointment.

For exams scheduled on: Saturday, Sunday and Monday, call Pearson VUE no later than Friday at least 1 full business day in advance of the time of your appointment.

7. Present one form of acceptable identification and your ATT on the day of the examination.
The only acceptable forms of identification in test centers in the U.S., American Samoa, Guam, Northern Mariana Islands and Virgin Islands are:

U.S. drivers license (not a temporary or learners permit)
U.S. state identification
Passport

For all other test (international) centers, only a passport is acceptable.

All identification must be written in English, have a signature in English, be valid (not expired) and include a photograph.

Candidates with identification from a country on the U.S. embargoed countries list will not be admitted to test.

You will not be admitted to the examination without acceptable identification and your ATT.

If you arrive without these materials, you forfeit your test session and must re-register; this includes re-payment of the $200 registration fee.

8. Receive your NCLEX examination results from the board of nursing you applied for licensure with within one month from your examination date.
