SCVNPPI MEMBERSHIP GUIDELINES

Definition of SCVNNPI:
Society of Cardiovascular Nurse Practitioners of the Philippines Inc.

How to find SCVNNPI:
OFFICE HOURS: 8-5
LOCATION: PHILIPPINE HEART CENTER
ADDRESS: East Avenue, Quezon City, Philippines
EXACT LOCATION: HOSPITAL BUILDING

DIRECTIONS:
1. leave an ID with the ENTRANCE guard
2. upon entering the building ride on the elevator located @ your right
3. go to the 3rd floor
4. upon stepping out of the elevator turn right and go straight thru the corridor
5. it is the FIRST OFFICE @ your right with the OFFICE NAME of:a. Department of Special Areas & b. Department of Clinical Areas
6. upon entering JUST SAY: "magpapaMEMBERSHIP po".
7.head back the same way you came from
8. get back the ID you left at the EXIT guard

MEMBERSHIP REQUIREMENTS:
1. provide your PRC number and Photocopy of your PRC ID
2. you must have your CPR valid ID with you and a Photocopy of that ID
2.a. or a photocopy of BLS/ACLS/PALS certificates

SPECIAL NOTE:
RED CROSS - BLS is OK
NC-CLEX ACLS & PALS ID and/or certificates are OK
CCNAPI ACLS is OK
PHC - BLS, ACLS, PALS are OK
Other Hospital Provided Trainings of BLS, ACLS, PALS are OK
JUST MAKE SURE ALL YOUR DOCUMENTS ARE VALID UPON MEMBERSHIP

3. 1 piece 1x1 ID picture
4. PAY 200 pesos
5. Validity of the ID is 1 year
6. You then will receive a hard paper card like ID wherein you will paste your ID picture and laminate it yourself.

THINGS TO REMEMBER FOR THE FORM:
@IF you are NOT working - do NOT write anything under the EMPLOYMENT SECTION provided at the UPPER FRONT of the membership form
@IF you are NOT working - do NOT check the box provided at the UPPER BACK of the membership form
@IF you attended BLS, ACLS, PALS - CHECK the box provided at the UPPER BACK of the membership form that says attended 24 hours continued education or training related to cardiovascular topics.
@YOU will NEED to know the hours/days of each training you attended that is in relation with CARDIO there is a space provided at the BOTTOM FRONT of the membership form for this also make sure you know where you got your trainings from.

How to find CCNAPI:
OFFICE HOURS: 8-5

ADDRESS: East Avenue, Quezon City, Philippines
EXACT LOCATION: PHC Medical Arts Building

DIRECTIONS:
1. go straight in the building (for people with bags - there is a bag check)
2. upon entering the building turn left and ride on the elevator located @ your left
3. go to the 8th floor
4. upon stepping out of the elevator turn right and go straight thru the SHORT corridor
5. IMMEDIATELY turn left (CCNAPI office is the FIRST corner door)
6. head back the same way you came from

MEMBERSHIP REQUIREMENTS:
1. provide your PRC number
2. bring 1 piece 1x1 ID picture
3. fill in the membership form and remember to sign thier signature coder
3. PAY 300 pesos
5. Validity of the ID is 1 year
6. You then will receive a credit card like ID (membership id will be given on the spot)

How to find RENAP:
OFFICE HOURS: 8-5

EXACT LOCATION: HOSPITAL BUILDING

DIRECTIONS:
1. leave an ID with the ENTRANCE guard
2. upon entering the building ride on the elevator located @ your right
3. go to the 4th floor
4. upon stepping out of the elevator turn right and go straight thru the corridor
5. at the end turn right (THIS IS SECTION 4A of the HOSPITAL BUILDING)
6. upon entering 4A which is a ward the location of RENAP OFFICE is behind the nurses' station
7. head back the same way you came from
8. get back the ID you left at the EXIT guard

MEMBERSHIP REQUIREMENTS:
1. create a list of references - personal or work related (It doesnt matter) - you will fill this out on their membership form.
2. provide your PRC number
3. NO need for ID picture
4. PAY 300 pesos
5. Validity of the ID is 1 year
6. You then will receive a hard paper card like ID wherein you will fill-in your name & signature
